Youtube or Vimeo for me. Your server won't serve video better than the big boys. If you want to have them locally though, I'd use JW Player and get them in MP4 format.

It isn't usual to list keywords in the HTML itself - in fact it can damage your ranking.

Are these simply to increase search chances?

Yep. I stupidly hadn't included my headers and therefore was calling functions that didn't exist or any connection to the database... N00b.

Hence why it worked fine with HTML but not dynamic...

It will be something in the theme you're modifying. Have you you copied this code from another form? it seems odd that the inputs have the same name and class....

Other than that it could be javascript, is this within your own site or software? (Wordpress / Drupal etc)
