I have no been able to fix this form... for some reason I receive the email... but the information from the form is not there... I dont know if there is a problem with my php docuemt or with the form itself.

leave that form as it is, meaning it should be the way you got it.

what you're suppose to change is the "form0#configuration.xml"
their should be instructions, & bla bla :\
