View Full Version : Make Form Appear with Yes or No response

I have made a form in Dreamweaver, and I understand the basics of creating forms. But I've been asked to try and create a form where the users will either check "yes" or "no" and if they check "yes" another box (like a text field) will appear for them to write in.

Is there a way for this to be done? And if so, how!? :? Thanks for any help!!

That is EXACTLY what I want to do, but it's not working for me. I'm sure I need some source files (like jquery-latest.pack.js?) but where do I get that?

Nevermind! I did a search for that file and uploaded it to my server and it works perfectly. THANKS!!
